Benevolence Request Form

The Benevolence Request Form can be utilized by the church when a member has a hardship or emergency and asks for financial help from the church. Once submitted, the form will be reviewed by the CITS Deacons. A member of the deacon team will then meet with the individual for additional information, prayer support, and to discuss next steps.

We ask that you include all relevant details related to your request. The information you provide will help us determine how we can best assist you. Sensitive information related to your request will only be shared with the necessary members of the deacon and elder teams at Church in the Square. Please be sure to specify the estimated amount requested, whether it's a specific figure or an estimated range. If applicable, include an estimated timeline for your benevolence request. Also, indicate if this is a one-time request or if it will likely be a recurring need.
